Parish Registration Form

For your convenience the Parish Registration Form is available in two formats: Microsoft Word or Adobe PDF.

• MS Word Format -- If you have access to MS Word, you may fill the forms out electronically and e-mail it to the parish. Please note there are four Word documents in a Zip file.
   

• Adobe PDF Format -- Using the free Adobe Reader, you may print the form, complete it by hand and mail it to the parish office:
